    Summary

Kidney biopsies in lupus nephropathy reveal disease severity correlates with clinical findings. Even without symptoms, significant lupus nephritis can be present, underscoring the importance of renal biopsy.

    Area of Science:

    • Nephrology
    • Rheumatology
    • Immunopathology

    Background:

    • Systemic lupus erythematosus (SLE) frequently affects the kidneys, leading to lupus nephropathy.
    • Accurate classification of lupus nephropathy is crucial for prognosis and treatment.

    Purpose of the Study:

    • To review the histological classification and clinical correlation of lupus nephropathy.
    • To assess the utility of renal biopsy in diagnosing lupus nephropathy, even in asymptomatic patients.

    Main Methods:

    • Retrospective review of 70 renal biopsies from 56 patients with SLE over 15 years.
    • Classification of lupus nephritis based on light microscopy: mesangial, focal, diffuse, or membranous.
    • Immunofluorescence and electron microscopy were utilized for detailed analysis.

    Main Results:

    • All biopsies showed positive immunofluorescence, including mesangial disease.
    • Electron microscopy revealed deposit location correlating with disease severity.
    • Clinical renal abnormalities were uncommon in mesangial disease but frequent in focal and diffuse lupus nephritis.

    Conclusions:

    • Renal biopsy is essential for diagnosing lupus nephropathy, as significant disease can exist without clinical symptoms.
    • Histological classification aids in understanding disease progression and guiding patient management.
