    Background:

    • Herpes gestationis (HG), now known as Pemphigoid Gestationis, is a rare autoimmune blistering disease of pregnancy.
    • Previous reports of HG in Black patients are exceptionally rare, with only two cases documented.
    • Understanding the genetic and immunological underpinnings of HG is crucial for diagnosis and management.

    Observation:

    • Two Black women presented with clinical and historical features typical of herpes gestationis.
    • Direct immunofluorescence microscopy revealed complement deposition at the dermoepidermal junction in both patients.
    • Immunogenetic analysis in one patient identified HLA-DR3 and HLA-DR4, along with anti-HLA-DR2 antibodies.

    Findings:

    • The observed HLA-DR3-DR4 combination, strongly associated with HG in White populations, was present in one Black patient.
    • The rarity of HLA-DR4 in American Black individuals may contribute to the infrequent reporting of HG in this demographic.
    • Elevated DR2 antigen in husbands of HG patients, particularly those with the DR3-DR4 combination, was noted.

    Implications:

    • Positive immunofluorescence findings and specific human leukocyte antigen (HLA) associations suggest that herpes gestationis is pathogenically consistent across different racial groups.
    • These findings challenge assumptions about racial predilection in HG and highlight the need for broader genetic association studies.
    • Further research into HLA associations in diverse populations can refine our understanding of HG pathogenesis and diagnostic markers.
