    Background:

    • Vertebral osteomyelitis is a challenging diagnosis.

    Observation:

    • Two cases of pyogenic vertebral osteomyelitis following UTIs are presented.
    • The condition exhibited subtle clinical signs and delayed radiographic progression.
    • Pre-existing degenerative arthritis complicated the diagnostic process.

    Findings:

    • Diagnosis was significantly delayed in both described cases.
    • Delayed appearance of radiologic signs contributed to diagnostic delays.
    • Escherichia coli was identified as the causative agent in UTIs preceding osteomyelitis.

    Implications:

    • Increased clinical awareness of vertebral osteomyelitis is crucial.
    • Radionuclide scanning can aid in earlier detection.
    • Prompt diagnosis can lead to better patient outcomes.
