    Background:

    Purpose of the Study:

    • To investigate the prevalence of abnormal glucose and insulin values in patients with suspected metabolic inner ear disorders.
    • To identify the primary metabolic abnormality contributing to these disorders.
    • To explore the link between hyperinsulinemia and Meniere's disease.

    Main Methods:

    • A 5-hour glucose tolerance test with simultaneous insulin titration was performed on 100 consecutive patients.
    • Analysis of glucose and insulin levels to identify abnormalities.
    • Clinical assessment for Meniere's disease.

    Main Results:

    • 82% of patients exhibited abnormal glucose and/or insulin levels.
    • Hyperinsulinemia was the most frequent abnormality, identified as a key factor in inner ear disorders.
    • 41% of cases showed hypo- or hyperglycemia; all patients had normal fasting glucose.
    • 26 patients were diagnosed with Meniere's disease.

    Conclusions:

    • Hyperinsulinemia is a significant factor in metabolic inner ear disorders, including Meniere's disease.
    • A diet low in simple carbohydrates is an effective management strategy for affected patients.
    • Further research into the pathophysiological model of insulin-induced Meniere's disease is warranted.
