    Summary

    Acne fulminans, a severe skin condition, can cause joint pain (polyarthralgia). This case study shows nonsteroidal anti-inflammatory drugs effectively managed joint pain in a young man with this condition.

    Background:

    • Acne fulminans is a rare, acute ulcerative acneiform eruption.
    • It is characterized by sudden onset of inflammatory papules, pustules, and nodules, often accompanied by systemic symptoms like fever and arthralgia.
    • The exact etiology remains unclear, but an immune-mediated process is suspected.

    Observation:

    • A 19-year-old male presented with acne fulminans.
    • He experienced prolonged polyarthralgia (joint pain affecting multiple joints).
    • The patient's joint pain was effectively managed with nonsteroidal anti-inflammatory agents.

    Findings:

    • Human Leukocyte Antigen (HLA) typing revealed the presence of A2, B7, and B8 antigens in the patient.
    • These HLA antigens may be associated with the susceptibility or presentation of acne fulminans and its associated arthralgia.
    • The positive response to nonsteroidal anti-inflammatory agents suggests an inflammatory component to the polyarthralgia.

    Implications:

    • This case highlights the importance of considering rheumatological manifestations in acne fulminans.
    • Nonsteroidal anti-inflammatory agents can be a valuable therapeutic option for managing polyarthralgia in acne fulminans.
    • Further research into the immunogenetic factors, including HLA associations, may elucidate the pathogenesis of acne fulminans and its systemic complications.
