Techniques for right and left ventricular endomyocardial biopsy.

Summary

Right ventricular endomyocardial biopsy via the internal jugular vein is safe and effective, yielding adequate tissue in over 98% of patients with low morbidity. Retrograde left ventricular biopsy is a reliable alternative.

Background:

Endomyocardial biopsy is crucial for diagnosing cardiac conditions.
Percutaneous approaches are preferred for safety and efficacy.

Purpose of the Study:

To evaluate the safety and efficacy of percutaneous right internal jugular approach for right ventricular endomyocardial biopsy.
To compare this approach with other methods, including retrograde left ventricular biopsy.

Main Methods:

Over 1,300 percutaneous right internal jugular right ventricular endomyocardial biopsies were performed.
Meticulous attention to methodologic detail was emphasized for both right and left heart biopsies.

Main Results:

The right internal jugular approach was safe and easily performed in over 1,300 procedures.
Adequate biopsy tissue was obtained in more than 98% of patients.
The morbidity rate associated with this technique was remarkably low.

Conclusions:

Percutaneous right internal jugular right ventricular endomyocardial biopsy is a safe, effective, and easily performed procedure.
Retrograde left ventricular endomyocardial biopsy is the safest and most reliable alternative.
Success of cardiac biopsy techniques relies on strict adherence to methodologic details.
