How much physical therapy for patients with stroke?

J C Brocklehurst, K Andrews, B Richards

    British Medical Journal
    |May 20, 1978

    Summary

This study compared physiotherapy, occupational therapy, and speech therapy for stroke patients. Physiotherapy was most common but did not prevent shoulder complications, suggesting a need to redefine its objectives.

    Area of Science:

    • Neurology
    • Rehabilitation Medicine
    • Clinical Therapy

    Background:

    • Stroke rehabilitation involves physiotherapy, occupational therapy, and speech therapy.
    • Limited data exists on the comparative effectiveness and patient allocation for these therapies post-stroke.

    Purpose of the Study:

    • To investigate the utilization of physiotherapy, occupational therapy, and speech therapy in stroke survivors.
    • To compare the outcomes and patient characteristics associated with each therapy.

    Main Methods:

    • Retrospective analysis of 135 stroke survivors at two weeks post-stroke.
    • Documented the number of patients receiving each therapy (physiotherapy, occupational therapy, speech therapy).
    • Assessed patient disability, prognosis, and development of shoulder complications.

    Main Results:

    • 107 patients received physiotherapy, 35 occupational therapy, and 19 speech therapy.
    • Patients receiving the most physiotherapy had higher disability and poorer prognosis.
    • Physiotherapy did not prevent shoulder stiffness or pain, which affected a significant number of patients.

    Conclusions:

    • The objectives of physiotherapy for stroke patients require clear definition, focusing on early intervention.
    • Continuation of therapy beyond six months, with limited recovery, warrants re-evaluation.
    • Alternative or simplified treatment approaches may be beneficial.
