[Preventive chemotherapy of intrapulmonary residual tuberculosis with isoniazid--example of failed compliance by the
Abstract:
Compliance with the recommendations of the Swiss Society for Tuberculosis and Lung Diseases regarding the treatment of patients with fibrotic pulmonary lesions with isoniazid was evaluated over the period March 1981-October 1982. 42 patients (mean age 53 +/- 16 years) were selected for preventive treatment. Diagnostic procedures were correctly performed in 22 patients (52.4%). 32 patients (76.2%) received the recommended drug initially. In 18 patients (42.9%) isoniazid was correctly prescribed for six months. In only 15 patients (35.7%) was the diagnostic and therapeutic approach correct. The reasons for these quality inadequacies on the diagnostic and therapeutic levels are discussed.
Insights
Compliance with Swiss guidelines for treating fibrotic pulmonary lesions with isoniazid was suboptimal. Many patients did not receive correct diagnosis or the full six-month treatment course, indicating quality gaps in care.
Area of Science:
- Pulmonology
- Infectious Diseases
- Pharmacotherapy
Context:
- Evaluation of treatment compliance for fibrotic pulmonary lesions.
- Focus on isoniazid preventive therapy recommendations from the Swiss Society for Tuberculosis and Lung Diseases.
- Study period: March 1981 - October 1982.
Purpose:
- To assess adherence to established guidelines for treating patients with fibrotic pulmonary lesions.
- To identify quality inadequacies in the diagnostic and therapeutic management of these patients.
- To analyze reasons for deviations from recommended treatment protocols.
Summary:
- 42 patients with fibrotic pulmonary lesions received preventive treatment.
- Diagnostic procedures were correctly performed in 52.4% of patients.
- Only 35.7% of patients had a fully correct diagnostic and therapeutic approach, with issues in drug prescription duration and initial drug selection.
Impact:
- Highlights significant quality gaps in the management of fibrotic pulmonary lesions.
- Underscores the need for improved adherence to diagnostic and therapeutic guidelines.
- Provides insights into potential areas for quality improvement in tuberculosis and lung disease treatment protocols.
