    Background:

    • Severe emotional disturbance in children requires accurate diagnosis and effective treatment.
    • Distinguishing emotional disturbance from attention deficit disorder (ADD) is crucial for appropriate intervention.

    Observation:

    • A six-year-old girl presented with severe emotional disturbance.
    • Her symptoms were not characteristic of attention deficit disorder, lacking issues with attention span or distractibility.

    Findings:

    • A double-blind, placebo-controlled study evaluated magnesium pemoline.
    • The study confirmed a beneficial pharmacologic response to magnesium pemoline in the child.

    Implications:

    • Magnesium pemoline may be an effective treatment for specific types of severe emotional disturbance in children.
    • Combined pharmacologic treatment with psychotherapy can be a viable approach for managing pediatric emotional disorders.
