    Area of Science:

    • Ophthalmology and Virology: Focuses on viral infections affecting the eye and surrounding skin structures.

    Background:

    • Orf virus (paravaccinia virus) causes contagious pustular dermatitis in sheep and goats.
    • Human orf virus infections are zoonotic, occurring through direct contact with infected animals.

    Observation:

    • A farmwife presented with a pericanthal eyelid lesion, follicular conjunctivitis, and regional lymphadenopathy after handling lambs.
    • Clinical presentation mimicked other infectious or inflammatory ocular conditions.

    Findings:

    • Electron microscopy confirmed the presence of orf virus.
    • The patient exhibited a raised, ulcerating lesion at the inner canthus and conjunctival inflammation.

    Implications:

    • Topical idoxuridine (35% in dimethylsulfoxide for skin, 0.5% ointment for conjunctiva) proved effective.
    • Prompt diagnosis and targeted antiviral therapy can lead to successful resolution of human orf virus infections.
    • Highlights the importance of considering zoonotic viral infections in individuals with relevant animal exposure.
