    Background:

    • Two patients showed dramatic recovery after unilateral ventriculostomy.

    • Unilateral ventriculostomy is an acceptable emergency approach for third ventricular colloid cysts.
    • Computed tomographic (CT) scanning aids in early detection of these cysts.
    • Ventriculoperitoneal shunting is an alternative to surgical cyst removal.

    • Unilateral ventriculostomy can be a life-saving emergency intervention.
    • CT scanning plays a vital role in the timely identification of colloid cysts.
