    Myasthenia gravis (MG) patients show increased immunoglobulin-secreting cells in the thymus. The thymus supports B-cell differentiation, suggesting it may be a site of heightened B-cell activation in MG.

    Background:

    • Myasthenia gravis (MG) is an autoimmune neuromuscular disease.
    • The thymus plays a crucial role in T-cell development and has been implicated in MG pathogenesis.
    • B-cell activity within the thymus in MG is not fully understood.

    Purpose of the Study:

    • To investigate immunoglobulin secretion by thymus and blood mononuclear cells in MG patients and controls.
    • To compare spontaneous and stimulated B-cell responses in the thymus and blood.
    • To assess the thymus as a potential site for B-cell activation in MG.

    Main Methods:

    • Used protein A reverse hemolytic plaque assay to quantify immunoglobulin-secreting cells (IgSC) for IgG, IgM, and IgA.
    • Employed ELISA to measure IgG levels in culture supernatants.
    • Studied freshly isolated and pokeweed mitogen (PWM)-stimulated cells from MG patients and cardiac surgery controls.

    Main Results:

    • MG thymus cells exhibited increased spontaneous immunoglobulin secretion compared to controls.
    • Both MG and control thymus cells responded to PWM stimulation, indicating B-cell differentiation capacity.
    • PWM-induced B-cell responses were sometimes greater in thymic cells than blood cells, despite lower B-cell frequency in the thymus.

    Conclusions:

    • The thymus provides a supportive environment for B-cell differentiation.
    • Elevated numbers of resident IgSC in the MG thymus suggest accentuated in vivo B-cell activation.
    • The thymus may be a key site for B-cell abnormalities in myasthenia gravis.
