    Background:

    • Menière's disease is characterized by endolymphatic hydrops, leading to auditory and vestibular symptoms.
    • Electrocochleography (ECoG) can reveal abnormalities in endolymphatic hydrops, such as an elevated summating potential (SP) to action potential (AP) ratio.
    • Glycerol is a hyperosmotic agent used to temporarily reduce endolymphatic fluid volume.

    Observation:

    • A single patient with Menière's disease and endolymphatic hydrops underwent glycerol dehydration.
    • Electrocochleography recordings were stable and demonstrated a significantly large SP in relation to the AP.
    • Pure tone and speech audiometry showed dramatic hearing improvement post-dehydration.

    Findings:

    • Following glycerol dehydration, the electrocochleogram showed a marked decrease in SP amplitude.
    • The AP amplitude remained relatively unchanged, leading to a near-normalization of the SP/AP waveform.
    • The observed ECoG changes correlated with the clinical improvement in hearing.

    Implications:

    • This case highlights the diagnostic utility of ECoG in identifying endolymphatic hydrops.
    • The findings suggest that changes in endolymphatic pressure significantly impact ECoG parameters.
    • Demonstrates the potential of dehydrating agents in managing Menière's disease symptoms and ECoG abnormalities.
