    Area of Science:

    • Toxicology
    • Clinical Pharmacology

    Background:

    • Clonidine is an imidazoline derivative commonly used to treat hypertension.
    • Acute intoxication can occur with excessive ingestion of clonidine.

    Observation:

    • A 60-year-old woman ingested 20 mg of clonidine, leading to acute intoxication.
    • Clinical presentation included central nervous system (CNS) depression, characterized by bradycardia and hypotonia.
    • Systemic hypertension and peripheral vasoconstriction were also observed.

    Findings:

    • The patient exhibited a paradoxical combination of CNS depression and systemic hypertension.
    • Treatment involved atropine to manage bradycardia and sodium nitroprusside for hypertension.
    • The patient demonstrated a positive response to the administered treatments.

    Implications:

    • This case highlights the complex cardiovascular and neurological effects of acute clonidine overdose.
    • Effective management requires addressing both the central and peripheral manifestations of toxicity.
    • Timely and appropriate medical intervention can lead to favorable outcomes in clonidine intoxication.
