    Summary

    Allergic contact dermatitis occurred in patients using Synthaderm, a polyurethane leg ulcer covering. Patch tests showed sensitivity to Synthaderm itself, not its components.

    Background:

    • Allergic contact dermatitis (ACD) is an immune response to contact allergens.
    • Polyurethane-based dressings are used for wound care, including leg ulcers.

    Observation:

    • Two patients developed ACD while using Synthaderm, a polyurethane covering for leg ulcers.
    • Patch testing was performed on one patient using Synthaderm extracts and isocyanate compounds.

    Findings:

    • The patient's patch tests were negative for Synthaderm extracts and isocyanates.
    • However, the patient showed positive reactions to Synthaderm samples from various batches and production years.

    Implications:

    • This suggests that the finished polyurethane material, rather than specific chemical components like isocyanates, may be responsible for allergic reactions.
    • Further investigation into the composition and manufacturing of polyurethane wound dressings is warranted to identify the causative agents in ACD.
