    Summary

Neutropenic typhlitis, an acute cecum inflammation in neutropenic patients, requires bowel rest, fluids, and antibiotics. Surgery is reserved for persistent symptoms post-hematologic recovery, with cecostomy preferred over hemicolectomy.

    Area of Science:

    • Gastroenterology
    • Oncology
    • Infectious Diseases

    Background:

    • Neutropenic typhlitis (NT) is a severe gastrointestinal complication in neutropenic patients.
    • Early recognition and management are crucial for patient outcomes.

    Observation:

    • Presents two cases of neutropenic typhlitis with characteristic clinical and radiological findings.
    • Highlights the typical presentation of acute cecal inflammation in immunocompromised individuals.

    Findings:

    • Conservative management including bowel rest, fluid resuscitation, and broad-spectrum antibiotics is the primary treatment.
    • Surgical intervention is indicated only if abdominal signs persist after hematologic recovery.
    • Cecostomy with drainage is effective for localized necrosis; right hemicolectomy is reserved for extensive necrosis.

    Implications:

    • This approach aims to minimize surgical risks in a vulnerable patient population.
    • Emphasizes the importance of a staged surgical strategy based on disease severity and patient recovery.
    • Provides a guideline for managing neutropenic typhlitis, balancing medical and surgical interventions.
