The pancreas: a correlation of function and structure.

J D Waye, M Adler, D A Dreiling

    The American Journal of Gastroenterology

    Summary

Secretin tests are sensitive for chronic pancreatitis, but combining them with pancreatography improves diagnosis. Together, these pancreatic function tests enhance accuracy and understanding of pancreatic diseases.

    Area of Science:

    • Gastroenterology
    • Diagnostic Imaging
    • Pancreatic Diseases

    Background:

    • Chronic pancreatitis diagnosis can be challenging.
    • Accurate differentiation between pancreatic conditions is vital for effective treatment.

    Purpose of the Study:

    • To evaluate the diagnostic utility of the secretin test and endoscopic retrograde pancreatography.
    • To determine the combined diagnostic value of these pancreatic function tests.

    Main Methods:

    • Fifty-two patients underwent secretin testing and endoscopic retrograde pancreatography.
    • Histopathological analysis was performed in half of the patients via exploratory laparotomy.

    Main Results:

    • Secretin tests demonstrated high sensitivity for detecting chronic pancreatitis.
    • Pancreatography proved essential for distinguishing malignant obstruction from pancreatic fibrosis or atrophy when secretory tests yielded low output.
    • Histological findings often showed discordance with ductular structure and secretin function.

    Conclusions:

    • Secretin tests are sensitive but not exhaustive for diagnosing chronic pancreatitis.
    • Combining secretin tests with pancreatography increases diagnostic accuracy.
    • These complementary pancreatic function tests offer a better understanding of pancreatic pathophysiology.
