    Background:

    • Extrinsic allergic alveolitis, or pigeon breeder's lung, is often associated with avian dust exposure.
    • High antibody titers against pigeon antigens are typically observed in affected individuals.

    Observation:

    • In some symptomatic individuals exposed to avian dust, antibodies were not initially demonstrable.
    • Following controlled inhalation challenges, low antibody titers appeared temporarily and then disappeared upon allergen avoidance.
    • Elevated cell-mediated immunity, measured by the lymphocyte transformation test, also decreased after allergen avoidance.

    Findings:

    • Antibody presence alone may not be the sole indicator for diagnosing pigeon breeder's lung.
    • Cell-mediated immunity, specifically T cell-dependent reactivity, is implicated in the disease pathogenesis.
    • Inhalation challenges can identify sensitive individuals who may not have detectable antibodies.
