    Summary

    Ascitic fluid in the pleural space without abdominal ascites is rare. Two patients with diaphragmatic defects showed this condition, which was diagnosed and surgically treated.

    • Both patients had diaphragmatic defects: a traumatic tear and a spontaneous perforation.

    Findings:

    • The study uniquely diagnosed transdiaphragmatic fluid movement during life.
    • Surgical correction of the diaphragmatic defects was successful.

    Implications:

    • Highlights rare cases of diaphragmatic defects causing pleural effusions.
    • Provides insights into the diagnosis and surgical management of transdiaphragmatic fluid movement.
