    Summary

    Retroperitoneal perforation during duodenoscopic sphincterotomy is a potential complication. However, conservative management with adequate biliary drainage is effective for most patients, as evidenced by a low incidence in this study.

    Background:

    • Duodenoscopic sphincterotomy is a common procedure for managing biliary and pancreatic diseases.
    • Perforation into the retroperitoneal space is a known, albeit uncommon, complication.

    Observation:

    • The study reviewed five cases of retroperitoneal perforation during approximately 500 duodenoscopic sphincterotomies.
    • Radiographs were used to identify and confirm the perforation.

    Findings:

    • Most patients with retroperitoneal perforation responded well to conservative management.
    • Adequate biliary drainage was crucial for successful conservative treatment.
    • Radiographic follow-up of 50 additional procedures showed no evidence of perforation, suggesting a low incidence or effective prevention/management.

    Implications:

    • Conservative management is a viable approach for retroperitoneal perforation post-sphincterotomy.
    • Ensuring adequate biliary drainage is key to successful outcomes.
    • The findings support the safety and efficacy of duodenoscopic sphincterotomy when complications are managed appropriately.
