    • These ocular symptoms were the sole evidence of a recurrent epidural hematoma.

    • The study reviews existing literature on the mechanisms linking extracerebral collections to ocular signs.
    • A novel mechanism is proposed to explain the development of these specific symptoms.

    • Understanding these mechanisms can improve the diagnosis and management of patients with epidural hematomas.
