    Summary

    Small intestinal mucosal changes were found in 20% of infants who died post-perinatal. Bottle-fed male infants had a higher incidence, suggesting feeding practices may influence these findings.

    Background:

    • Small intestinal mucosal changes have been observed in post-perinatal death cases.
    • Previous studies linked these lesions to heatstroke, particularly in sudden infant death syndrome.

    Purpose of the Study:

    • To investigate the prevalence and potential causes of small intestinal mucosal changes in post-perinatal deaths.
    • To determine associations with feeding methods, sex, and cause of death.

    Main Methods:

    • Examination of small intestinal mucosa from 78 post-perinatal death cases in Sheffield (Sept 1980 - Sept 1981).
    • Analysis of data concerning explained vs. unexplained deaths, viral isolation, feeding practices, and infant sex.

    Main Results:

    • Mucosal changes were present in 20% (18/78) of cases before death.
    • Lesions were more common in males and strongly associated with bottle feeding; no exclusively breast-fed infants showed enteropathy.
    • No significant difference in prevalence between explained and unexplained deaths or association with viral isolation.

    Conclusions:

    • Bottle feeding is strongly associated with infant small intestinal enteropathy.
    • While heatstroke was previously suggested, the findings in explained deaths complicate this theory, though overheating remains a possibility for home deaths.
