    • A 69-year-old patient developed severe diffuse interstitial pneumonia after five weeks of gold salt treatment.
    • Symptoms improved upon discontinuation of gold salts and initiation of steroid therapy.

    • Transbronchial biopsy and bronchoalveolar lavage fluid analysis provided detailed pathological and cellular data.
    • Findings supported a diagnosis of gold salt-induced interstitial pneumonia, likely a cell-mediated hypersensitivity reaction.

    Implications:

    • This case highlights the importance of considering gold salt toxicity in patients presenting with interstitial pneumonia.
    • Further research into the immunopathogenesis of drug-induced lung injury is warranted.
