    A rare case of leprosy (a bacterial infection) presented as an unusual breast lesion in a US woman. Pathological findings revealed nerve inflammation and bacterial presence, suggesting an indeterminate leprosy form.

    Background:

    • Leprosy, a chronic infectious disease caused by Mycobacterium leprae, typically affects the skin, peripheral nerves, and upper respiratory tract.
    • While endemic in certain regions, leprosy can occur in non-endemic areas, presenting diagnostic challenges.
    • Early diagnosis and treatment are crucial to prevent nerve damage and disability.

    Observation:

    • A 62-year-old woman from a non-endemic US region presented with a progressively enlarging, anesthetic lesion covering her left breast.
    • Microscopic examination of skin biopsy revealed chronic perineural inflammation and neural proliferation.
    • An acid-fast bacillus, indicative of leprosy, was identified within a nerve sample.

    Findings:

    • The biopsy did not show granulomas, giant cells, epithelioid cells, or nerve abscesses, which are characteristic of tuberculoid leprosy.
    • Pathological examination suggested an indeterminate form of leprosy.
    • A notable feature was the significant degree of neural proliferation associated with the lesion.

    Implications:

    • This case highlights the importance of considering leprosy in the differential diagnosis of atypical skin lesions, even in non-endemic areas.
    • The unusual presentation and pathological findings underscore the diverse clinical spectrum of leprosy.
    • Further research into the atypical manifestations and pathogenesis of leprosy is warranted.
