    Iris leakage after cataract surgery is common, but cystoid macular edema (CME) is less frequent with intraocular lens (IOL) implantation after extracapsular cataract extraction (ECCE). CME presence correlates with iris leakage.

    Background:

    • Cataract surgery can lead to postoperative complications.
    • Iris leakage and cystoid macular edema (CME) are potential concerns following such procedures.

    Purpose of the Study:

    • To determine the incidence of CME and iris leakage post-cataract surgery.
    • To investigate the relationship between iris leakage and CME.
    • To assess the impact of surgical technique and intraocular lens (IOL) implantation on these outcomes.

    Main Methods:

    • An angiographic study was conducted on 121 implanted eyes and 94 aphakic eyes.
    • Evaluations were performed at 15 days, three months, six months, and 12 months postoperatively.
    • Iris and retinal vasculature were analyzed for leakage and edema.

    Main Results:

    • Iris leakage was a common finding in the immediate postoperative period.
    • The type of cataract extraction (extracapsular vs. intracapsular) did not significantly influence iris angiographic findings.
    • Cystoid macular edema (CME) incidence at three months was significantly lower (P < 0.05) in patients with an intraocular lens (IOL) following extracapsular cataract extraction (ECCE) compared to intracapsular cataract extraction (ICCE).
    • The presence of CME was closely related to angiographically observed iris leakage.

    Conclusions:

    • Iris leakage is a frequent early complication of cataract surgery.
    • Extracapsular cataract extraction (ECCE) with intraocular lens (IOL) implantation is associated with a reduced incidence of cystoid macular edema (CME) compared to intracapsular cataract extraction (ICCE).
    • Angiographically confirmed iris leakage is a significant predictor of cystoid macular edema (CME) post-cataract surgery.
