    Summary

    Morbidly obese patients undergoing gastric bariatric surgery have a high incidence of biliary tract disease, approximately four times that of the general population. Intraoperative ultrasonography aids in detecting this occult disease.

    Background:

    • Morbid obesity is a significant risk factor for biliary tract disease.
    • Bariatric surgery is increasingly performed in morbidly obese patients.
    • The prevalence of gallbladder disease in this population is not well-defined.

    Purpose of the Study:

    • To determine the incidence of biliary tract disease in morbidly obese patients undergoing gastric bariatric surgery.
    • To evaluate the utility of intraoperative ultrasonography in detecting occult biliary tract disease.

    Main Methods:

    • Retrospective review of 477 morbidly obese patients who underwent gastric bariatric surgery over a five-year period.
    • Analysis of the incidence of diagnosed biliary tract disease.
    • Intraoperative ultrasonic examination of the gallbladder in a subset of 50 patients.

    Main Results:

    • The incidence of biliary tract disease was 39.1% in the studied cohort.
    • This incidence is approximately four times higher than in the nonobese population.
    • Intraoperative ultrasonography proved effective in identifying occult biliary tract disease.

    Conclusions:

    • Morbidly obese patients undergoing bariatric surgery have a significantly elevated risk of biliary tract disease.
    • Routine screening for biliary pathology may be warranted in this patient group.
    • Intraoperative ultrasonography is a valuable tool for detecting asymptomatic gallbladder disease during bariatric procedures.
