    Area of Science:

    • Oncology
    • Pathology
    • Thoracic Surgery

    Background:

    • Thymic cysts are rare, benign mediastinal masses.
    • Malignant transformation of thymic cysts has not been previously documented.

    Observation:

    • A case of a patient presenting with a thymic cyst was observed.
    • The cyst was found to have undergone malignant transformation.

    Findings:

    • The first documented instance of well-differentiated papillary squamous cell carcinoma arising within a preexisting thymic cyst.
    • Surgical excision of the tumor was performed.
    • No evidence of metastasis was detected 6.5 years post-surgery.

    Implications:

    • This case expands the known spectrum of thymic lesions.
    • Highlights the importance of considering malignancy in complex thymic cysts.
    • Suggests surgical management can be effective for this rare condition.
