    Summary

    Diagnosing intestinal perforation can be challenging without free air. Computed tomography (CT) effectively identified leaks using oral contrast, aiding diagnosis in these challenging cases.

    • CT imaging provided definitive evidence of intestinal perforation.
    • CT facilitated accurate diagnosis when radiography was inconclusive.

    Implications:

    • CT with oral contrast is a valuable tool for diagnosing intestinal perforation when radiography is negative.
    • This imaging modality can improve diagnostic accuracy and timely surgical intervention.
    • Consider CT in patients with suspected perforation but no radiographic free air.
