    Summary
    This summary is machine-generated.

Herpes simplex virus can cause esophageal infections in immunocompromised patients. Radiography shows discrete ulcers, suggesting herpes esophagitis and avoiding unnecessary antifungal treatments.

    Area of Science:

    • Gastroenterology
    • Infectious Diseases
    • Radiology

    Background:

    • Herpes simplex virus (HSV) is an opportunistic pathogen that can infect the esophagus, particularly in immunocompromised individuals.
    • Radiographic features of HSV esophagitis are not well-documented, leading to potential diagnostic challenges.

    Observation:

    • This study reviewed six cases of HSV esophagitis in patients with clinical evidence of immunosuppression.
    • The most frequent symptom reported was acute odynophagia (painful swallowing).

    Findings:

    • Double-contrast esophagrams in these patients revealed characteristic findings such as discrete, widely separated ulcers.
    • Plaquelike defects, with or without ulceration, were also observed.
    • While plaquelike lesions can be seen in both herpes and monilial esophagitis, discrete ulcers on normal esophageal mucosa strongly suggest HSV esophagitis.

    Implications:

    • Improved recognition of these radiographic findings can enhance the diagnosis of herpes esophagitis.
    • Accurate diagnosis may help clinicians avoid the unnecessary use of potentially toxic antifungal medications in favor of appropriate antiviral therapy.
