[Myocardial infarction while windsurfing (author's transl)].

    Summary

Sudden cardiac death in a windsurfer was linked to undiagnosed Wolff-Parkinson-White syndrome. This condition can cause dangerous arrhythmias, leading to collapse and, in rare cases, myocardial infarction.

    Area of Science:

    • Cardiology
    • Sports Medicine
    • Pathology

    Background:

    • Undiagnosed Wolff-Parkinson-White syndrome can predispose individuals to paroxysmal tachycardias and sudden collapse.
    • The risks associated with pre-existing cardiac conditions during strenuous physical activity are significant.

    Observation:

    • A 40-year-old windsurfer collapsed and subsequently died.
    • ECG revealed a posterior wall infarction; autopsy confirmed both fresh and older infarctions.
    • Coronary arteries were unremarkable, suggesting non-atherosclerotic causes for infarction.

    Findings:

    • Indirect history suggested Wolff-Parkinson-White syndrome with a history of paroxysmal tachycardias and collapses.
    • The patient experienced a fatal posterior wall myocardial infarction despite unremarkable coronary arteries.

    Implications:

    • This case highlights the critical importance of identifying Wolff-Parkinson-White syndrome in athletes presenting with collapse.
    • It underscores the potential for cardiac events, including infarction, in individuals with undiagnosed accessory pathway syndromes during physical exertion.
    • Further investigation into the link between intense sports like windsurfing and reinfarction in susceptible individuals is warranted.
