    Summary

    Persistently elevated aspartate aminotransferase (AST) levels in serum, unexplained by disease, were found to be due to a benign complex of AST with immunoglobulin G (IgG). This finding can prevent unnecessary medical interventions and patient distress.

    Background:

    • Elevated serum aspartate aminotransferase (AST) activity is a common indicator of liver damage.
    • Unexplained elevations can lead to extensive and costly diagnostic evaluations.

    Observation:

    • Two patients presented with persistent, isolated, and unexplained elevations in serum AST activity.
    • These elevations persisted for years in one patient, leading to significant medical investigations and hospitalizations.

    Findings:

    • Extensive investigations failed to identify an underlying clinical cause for the elevated AST in both patients.
    • Electrophoretic and immunologic analyses revealed that the AST enzyme was complexed with immunoglobulin G (IgG) in their serum.
