    Summary

    Methyl-glyoxal-bis-guanylhydrazone (Methyl-GAG) is safe for anephric patients undergoing dialysis for metastatic hypernephroma. Dialysis effectively removes the drug, allowing for full dosage and rapid clearance if toxicity occurs.

    Background:

    • Metastatic hypernephroma presents challenges in treatment, particularly in patients with kidney failure.
    • Anephric patients requiring hemodialysis have altered drug metabolism and excretion pathways.
    • Methyl-glyoxal-bis-guanylhydrazone (Methyl-GAG) is an antineoplastic agent with potential use in advanced cancers.

    Observation:

    • A single anephric male patient with metastatic hypernephroma undergoing hemodialysis was administered low doses of Methyl-GAG.
    • Serum levels of Methyl-GAG were monitored to assess its pharmacokinetics in this patient population.
    • No adverse events or signs of toxicity were observed during the study period.

    Findings:

    • Methyl-GAG was found to be dialyzable, indicating efficient removal by hemodialysis.
    • The drug did not exhibit toxicity at the administered low doses in the anephric patient.
    • Serum concentration measurements confirmed the drug's clearance via dialysis.

    Implications:

    • Full therapeutic doses of Methyl-GAG may be permissible for anephric patients undergoing dialysis for metastatic hypernephroma.
    • Hemodialysis can be utilized as a method for rapid drug elimination in cases of potential Methyl-GAG toxicity.
    • This study supports the consideration of Methyl-GAG in renal failure patients, with careful management through dialysis.
