Bundle branch block in acute myocardial infarction.

G Hollander, V Nadiminti, E Lichstein

    American Heart Journal

    Summary

Patients with acute myocardial infarction and bundle branch block face high risks of heart block. Prophylactic pacemaker insertion is recommended for those with bifascicular block and anterior infarction, or left bundle branch block with anterior infarction.

    Area of Science:

    • Cardiology
    • Internal Medicine

    Background:

    • Bundle branch block complicates 8-13% of acute myocardial infarction cases.
    • Management guidelines for acute myocardial infarction with pre-existing bundle branch block are lacking.
    • High-degree atrioventricular block is a significant risk in these patients.

    Purpose of the Study:

    • To assess the risk of high-degree atrioventricular block in patients with acute myocardial infarction and bundle branch block.
    • To identify high-risk subgroups requiring specific interventions.
    • To inform therapeutic guidelines for this patient population.

    Main Methods:

    • Retrospective review of 606 patients with myocardial infarction admitted to a coronary care unit.
    • Analysis of 47 patients (8%) who presented with complete bundle branch block.

    Related Experiment Videos

  • Evaluation of progression to high-degree atrioventricular block based on infarction location and type of bundle branch block.
  • Main Results:

    • Patients with pre-existing bifascicular block and anterior wall infarction had a 25% risk of progression to high-degree atrioventricular block.
    • Patients with left bundle branch block and acute anterior wall infarction had a 100% risk of progression.
    • Significant risk identified in specific subgroups.

    Conclusions:

    • Prophylactic pacemaker insertion is recommended for patients with pre-existing bifascicular block and anterior wall infarction.
    • Prophylactic pacemaker insertion is strongly recommended for patients with left bundle branch block and acute anterior wall infarction.
    • These recommendations aim to mitigate the risk of severe atrioventricular block in high-risk acute myocardial infarction patients.
