    Summary

    This study suggests hypothyroidism and pseudotumor cerebri are not causally linked. Treatment with glucocorticoids resolved intracranial hypertension after thyroid normalization failed.

    Background:

    • Pseudotumor cerebri (idiopathic intracranial hypertension) is a condition of increased intracranial pressure.
    • Concurrent hypothyroidism and pseudotumor cerebri can present diagnostic and therapeutic challenges.

    Observation:

    • A 38-year-old obese woman with hypothyroidism and pseudotumor cerebri received levothyroxine therapy.
    • Thyroid function normalized, but intracranial hypertension persisted for over four months.

    Findings:

    • Standard treatments including weight loss, acetazolamide, and CSF drainage were ineffective.
    • Glucocorticoid therapy led to prompt and sustained resolution of pseudotumor cerebri.

    Implications:

    • This case challenges the causal relationship between thyroid hormone deficiency and pseudotumor cerebri.
    • Glucocorticoids may be an effective treatment for refractory pseudotumor cerebri in select cases.
