    Background:

    • Objective tinnitus is rare, often caused by palatal myoclonus or arteriovenous (AV) communications.
    • Diagnosing palatal myoclonus may involve tympanometry, electromyography (EMG), and electroencephalography (EEG).
    • Arteriovenous communications present as pulsatile tinnitus, potentially alleviated by pressure on the mastoid or carotid artery.

    Observation:

    • Standard audiometric and tympanometric tests are insufficient for diagnosing vascular causes.
    • Careful angiographic studies are crucial as routine tests may miss underlying vascular lesions.
    • A case suggests external carotid artery branch stenosis as a novel cause of vascular objective tinnitus.

    Findings:

    • Palatal myoclonus treatment, both medical and surgical, is frequently unsatisfactory.
    • Surgical intervention for arteriovenous communications typically resolves objective tinnitus symptoms.
    • Accurate diagnosis is paramount for effective treatment of objective tinnitus.

    Implications:

    • Recognizing external carotid artery stenosis expands the differential diagnosis for vascular objective tinnitus.
    • Improved diagnostic strategies are needed to identify the precise cause of objective tinnitus.
    • Targeted treatment based on accurate diagnosis offers the best prognosis for patients with objective tinnitus.
