    Summary

    A rare Moraxella-like organism (M6) caused endocarditis in a patient with mitral valve prolapse. While ampicillin treatment was effective, progressive heart failure necessitated valve replacement.

    Background:

    • Moraxella-like organism (M6) is typically pharyngeal and nonpathogenic.
    • Mitral valve prolapse is a known cardiac condition predisposing to endocarditis.

    Observation:

    • A 31-year-old female with mitral valve prolapse developed endocarditis.
    • The causative agent was identified as an M6 organism.

    Findings:

    • The endocarditis infection responded promptly to ampicillin therapy.
    • Despite antibiotic treatment, progressive left ventricular failure occurred.

    Implications:

    • This case highlights a rare cause of infective endocarditis.
    • Severe cardiac complications may necessitate surgical intervention even with appropriate antibiotic management.
