    Two patients with chronic ulcerative colitis developed painful pustular eruptions linked to severe colitis flares. These lesions may represent a variant of pyoderma gangrenosum, responding to immunosuppressants and colectomy.

    • Two patients with CUC developed painful pustules on skin and oral mucosa.

    • Infectious causes were ruled out via cultures.

    • Histology showed suppurative folliculitis.
    • Lesions resolved rapidly with corticosteroids, antibiotics, and colectomy in both cases.
