Prophylaxis of aphakic retinal detachment

R Le Mesurier, A H Chignell

Transactions of the Ophthalmological Societies of the United Kingdom
    |January 1, 1981
    PubMed
    Summary

Patients with aphakic retinal detachment in one eye have a lower risk of macula-affecting detachment in the second eye. Prophylactic cryotherapy is not recommended if follow-up care is available.

    Area of Science:

    • Ophthalmology
    • Retinal Diseases

    Background:

    • Aphakic retinal detachment is a known complication after cataract surgery.
    • Patients who have experienced detachment in one eye are at higher risk in the second eye.

    Purpose of the Study:

    • To evaluate the incidence and pattern of retinal detachment in the second eye of patients with prior aphakic detachment in the first eye.
    • To assess the impact of detachment in the second eye on central vision, particularly macula involvement.

    Main Methods:

    • Retrospective analysis of 88 patients with aphakic detachment in the first eye.
    • Monitoring of 57 aphakic second eyes for retinal detachment development and visual outcomes.

    Main Results:

    • Fifteen out of 57 (26.3%) second eyes developed retinal detachment.
    • Only two patients experienced a reduction in central vision due to macula detachment in the second eye.
    • This suggests a potentially altered pattern of disease in the second eye.

    Conclusions:

    • The risk of vision-threatening macula detachment appears lower in the second eye compared to the first.
    • Aggressive prophylactic treatment like 360-degree cryotherapy may not be necessary for all cases.
    • Close follow-up is crucial for monitoring patients at risk.
