    Summary

    This study compared Goldmann applanation tonometry and Pneuma-Tonometer for measuring intraocular pressure. The Pneuma-Tonometer showed good intra-reader reliability but requires calibration adjustments for accurate intraocular pressure assessment.

    Background:

    • Intraocular pressure (IOP) measurement is crucial for diagnosing and managing glaucoma.
    • Goldmann applanation tonometry is the gold standard, but alternative devices are sought for efficiency and accessibility.

    Purpose of the Study:

    • To evaluate the reliability and agreement of the Pneuma-Tonometer against the Goldmann applanation tonometer for IOP measurement.
    • To assess the intra-reader variability of the Pneuma-Tonometer.

    Main Methods:

    • 125 patients undergoing routine eye care had their IOP measured using both Goldmann applanation tonometer and Pneuma-Tonometer.
    • Pneuma-Tonometer readings were independently assessed by two blinded observers.
    • Statistical analysis included standard deviation of differences and correlation coefficients.

    Main Results:

    • The Pneuma-Tonometer demonstrated low intra-reader variability (SD=0.44 mm Hg).
    • Paired Pneuma-Tonometer readings showed higher variability (SD=1.75 mm Hg).
    • The Pneuma-Tonometer readings were, on average, 0.40 mm Hg higher than Goldmann readings, with a strong correlation (r=0.88) and a standard deviation of differences of 1.50 mm Hg.

    • The Pneuma-Tonometer exhibits good intra-reader reliability.
    • Calibration adjustments are necessary for the Pneuma-Tonometer to align with Goldmann applanation tonometer measurements.
    • Alternative calibration methods beyond linear regression may be beneficial.
