    Summary

    Recurrent hypercalcemia in T-cell lymphoma-leukemia may stem from tumor cell products activating osteoclasts. This suggests a novel mechanism beyond bone metastasis for calcium dysregulation in cancer.

    Area of Science:

    Background:

    • T-cell lymphoma-leukemia can present with hypercalcemia.
    • Hypercalcemia in cancer often relates to bone metastases.

    Observation:

    • Two adults with T-cell lymphoma-leukemia and hypercalcemia lacked bone disease.
    • Bone biopsies revealed significant osteoclast activation.
    • Malignant cells released bone-resorbing factors in vitro.

    Findings:

    • Tumor cells produced factors, including prostaglandin E, that activated osteoclasts.
    • Parathyroid hormone levels were normal, ruling out primary hyperparathyroidism.
    • These tumor products may be similar to osteoclast-activating factor.
