    Background:

    • Left ventricular aneurysmectomy is a surgical procedure to address aneurysms in the heart's main pumping chamber.
    • Preoperative assessment often involves left ventricular angiography and electrocardiography (ECG).

    Purpose of the Study:

    • To evaluate the long-term outcomes of left ventricular aneurysmectomy.
    • To assess changes in left ventricular function, ECG parameters, and the impact of coronary artery disease progression.

    Main Methods:

    • Retrospective analysis of 26 patients undergoing left ventricular aneurysmectomy.
    • Follow-up included left ventricular angiography and ECGs 9 to 62 months post-surgery.
    • Assessment of residual aneurysm size, ST segment elevation (sigma ST), and left ventricular end-diastolic pressure (LVEDP).

    Main Results:

    • Reduced mean sigma ST from 11.2 mm to 7.7 mm, but no normalization of ST segments.
    • Significant reduction in LVEDP from 21.5 mmHg to 15.1 mmHg pre-angiography and 26.7 mmHg to 21.1 mmHg post-angiography.
    • Progression of coronary artery stenoses correlated with unchanged LVEDP, while improved LVEDP was seen in patients without progression. Additional bypass grafting did not improve LVEDP.

    Conclusions:

    • Left ventricular aneurysmectomy can lead to improvements in ECG and LVEDP.
    • Progression of coronary artery disease is a significant factor in long-term left ventricular function deterioration after aneurysmectomy.
    • Further surgical interventions like bypass grafting may not consistently improve left ventricular function in this context.
