    Summary

    Meningeal carcinomatosis, often from breast or lung cancer, rarely shows improvement. Combination treatment offered some relief, but survival remained short for these advanced cancer cases.

    Background:

    • Meningeal carcinomatosis is a rare complication of advanced cancers.
    • Breast, lung carcinoma, and melanoma are common primary sites.
    • Diagnosis often occurs with widespread metastatic disease.

    Purpose of the Study:

    • To review cases of meningeal carcinomatosis.
    • To identify primary sources of the disease.
    • To evaluate treatment efficacy and patient outcomes.

    Main Methods:

    • Retrospective review of 33 patient cases.
    • Analysis of primary cancer types and metastatic spread.
    • Assessment of treatment responses to radiotherapy and intrathecal methotrexate.

    Main Results:

    • Breast carcinoma was the most frequent source (21 cases).
    • Lung carcinoma and malignant melanoma were also significant sources (5 cases each).
    • 78% of patients had widespread metastases at diagnosis.
    • Combination therapy (radiotherapy + intrathecal methotrexate) provided symptomatic improvement in 14/22 patients.
    • Mean survival in the most improved group was under six months.

    Conclusions:

    • Meningeal carcinomatosis is a severe complication of advanced cancers.
    • Combined treatment modalities offer limited symptomatic benefit.
    • Prognosis remains poor, with short survival times.
