    Summary

    Background:

    • Acute viral hepatitis can potentially affect multiple organ systems.
    • Renal involvement in viral hepatitis, particularly immune complex deposition, requires further investigation.

    Purpose of the Study:

    • To investigate the presence and characteristics of renal immune complexes in patients with acute viral hepatitis.
    • To correlate light microscopic findings in the kidney with immunofluorescence results.

    Main Methods:

    • Direct immunofluorescence was used to detect renal immune complexes in 22 patients with fatal acute viral hepatitis.
    • Light microscopy was performed on liver, kidney, and other organs.
    • Serum samples were analyzed for Hepatitis B surface antigen (HBsAg).

    Main Results:

    • Thirteen of 22 patients (59%) exhibited mild to moderate glomerular abnormalities on light microscopy.
    • Fifteen of 22 patients (68%) tested positive for renal immune complexes via immunofluorescence.
    • Immunoglobulin A (IgA) immune complexes were detected in 28% of patients. The incidence of HBsAg was low.

    Conclusions:

    • Acute viral hepatitis is associated with a high incidence of renal immune complex deposition.
    • Both Hepatitis B and non-B viral hepatitis types appear to contribute to glomerular morphological alterations and immune complex formation.
    • Renal immune complexes and glomerular changes are significant findings in fatal cases of acute viral hepatitis.
