    Background:

    • Alopecia areata is an autoimmune condition affecting hair follicles.
    • The role of anti-endothelial cell antibodies in alopecia areata pathogenesis is not fully understood.

    Purpose of the Study:

    • To investigate the presence and characteristics of anti-endothelial cell antibodies in patients with alopecia areata.
    • To explore the potential link between these antibodies and T cell counts.

    Main Methods:

    • Acid elution techniques were used to remove antibodies from circulating lymphocytes of eight alopecia areata patients.
    • Antibody binding specificity was assessed using immunofluorescence.
    • Circulating antinuclear antibodies (ANA) and T cell counts were also analyzed.

    Main Results:

    • Anti-endothelial cell antibodies were detected, specifically targeting endothelial cells in the hair bulb's capillary network.
    • These antibodies are unique to hair follicle endothelial cells, do not bind complement in vitro, and are species-specific.
    • Six out of eight patients showed a significant decrease in circulating T cells. Speckled ANA was observed in one case.

    Conclusions:

    • The findings suggest that anti-endothelial cell antibodies targeting hair follicle capillaries are present in alopecia areata.
    • These antibodies may contribute to the pathogenesis of alopecia areata.
    • A decrease in circulating T cells is associated with the condition in some patients.
