    Purpose of the Study:

    • To investigate the specific role of adrenergic signaling in counterregulating severe insulin-induced hypoglycemia.
    • To assess the impact of adrenergic blockade on glucose recovery and hormonal responses.

    Main Methods:

    • Single-blind randomized studies were conducted in normal male subjects.
    • Severe insulin-induced hypoglycemia was induced with or without adrenergic blockade (phentolamine or propranolol).
    • Plasma glucose, norepinephrine, lactate, pyruvate, and non-esterified fatty acid levels were monitored.

    Main Results:

    • Phentolamine did not impede euglycemia restoration.
    • Propranolol significantly retarded blood glucose recovery compared to controls, despite intact glucagon, epinephrine, cortisol, and growth hormone responses.
    • Propranolol blunted increases in lactate, pyruvate, and non-esterified fatty acids, while phentolamine augmented norepinephrine and rebound non-esterified fatty acids.

    Conclusions:

    • Full sympathetic nervous system activity is essential for complete blood glucose recovery from severe hypoglycemia.
    • Other counterregulatory mechanisms are insufficient alone when adrenergic pathways are blocked.
