Chlamydia trachomatis rectal infections were studied in homosexual men. Lymphogranuloma venereum (LGV) strains caused severe proctitis mimicking Crohn's disease, while non-LGV strains caused mild proctitis.

    Area of Science:

    • Proctology
    • Infectious Diseases
    • Gastroenterology

    Background:

    • Investigating the prevalence and clinical spectrum of Chlamydia trachomatis rectal infections in homosexual men.
    • Understanding the histopathological differences between LGV and non-LGV C. trachomatis infections.

    Observation:

    • 171 homosexual men were enrolled; 96 symptomatic and 75 asymptomatic.
    • C. trachomatis was isolated from 14 men.
    • LGV immunotypes were found in 3 symptomatic men with severe proctitis, showing granulomatous inflammation mimicking Crohn's disease.

    Findings:

    • LGV immunotypes of C. trachomatis are associated with severe acute proctitis.
    • Non-LGV immunotypes are linked to mild proctitis in both symptomatic and asymptomatic individuals.

    Related Experiment Videos

  • Fecal leukocytes and mild mucosal abnormalities were noted in non-LGV infections.
  • Implications:

    • LGV rectal infections can present as severe proctitis, potentially misdiagnosed as Crohn's disease.
    • Distinguishing LGV from non-LGV C. trachomatis is crucial for appropriate patient management.
    • Highlights the importance of STI screening in at-risk populations.
