Cutaneous necrosis caused by systemically administered heparin

    Summary

    Intravenous porcine heparin can cause skin necrosis, mimicking warfarin-induced necrosis but without fibrin thrombi. Patients tolerated warfarin treatment after discontinuing heparin.

    Background:

    • Deep venous thrombophlebitis is a serious condition requiring anticoagulation.
    • Porcine heparin and warfarin sodium are anticoagulants used in managing thrombophlebitis.
    • Cutaneous necrosis is a rare but severe adverse effect associated with anticoagulation therapy.

    Observation:

    • Two patients developed cutaneous necrosis following intravenous administration of porcine heparin for deep venous thrombophlebitis.
    • The skin lesions presented clinically similar to warfarin sodium-induced skin necrosis.
    • Histopathological examination revealed no fibrin thrombi in capillaries and vessels, distinguishing it from warfarin necrosis.

    Findings:

    • The observed cutaneous necrosis was attributed to porcine heparin, not warfarin.
    • Histology ruled out vasculitis as the cause of the lesions.
    • The absence of fibrin thrombi differentiated this heparin-induced necrosis from warfarin-induced necrosis.

    Implications:

    • This suggests a distinct mechanism for heparin-induced cutaneous necrosis compared to warfarin.
    • Clinicians should consider heparin as a potential cause of skin necrosis in patients receiving anticoagulation.
    • Alternative anticoagulation, such as warfarin, may be safely administered after heparin-induced necrosis.
