    Background:

    • Surgical trauma to the membranous labyrinth is a rare but severe complication of chronic ear surgery.

    • Common injury mechanisms include footplate subluxation, lateral canal fracture, and fistula formation.

    Observation:

    • Presents three cases of lateral semicircular canal labyrinthotomy during ear surgery.
    • Two injuries resulted from drilling, and one from cholesteatoma-related dehiscence.
    • Pre- and postoperative audiograms, vestibular tests, and photographs document the cases.

    • Notably, cochlear function remained intact in all three cases.
    • One patient experienced improved hearing post-injury.
    • The study refutes the assumption that labyrinthotomy invariably leads to a 'dead ear'.
