    This study reports a rare clear cell sarcoma in the Achilles tendon co-occurring with a deficiency in immunoglobulin M (IgM). Suppressor cells in the patient

    • Clear cell sarcoma of tendons and aponeuroses is a rare soft tissue malignancy.

    Observation:

    • A patient presented with clear cell sarcoma originating in the Achilles tendon, with widespread metastases.
    • The patient exhibited a complete absence of serum IgM, normal IgA, and elevated IgG levels.
    • Microscopic examination revealed glycogen-rich clear cells with both melanotic and amelanotic characteristics.

    Findings:

    • Peripheral blood lymphocytes showed a normal percentage of IgM-bearing cells.
    • In vitro studies demonstrated that the patient's mononuclear cells suppressed IgM production in normal lymphocytes.
