    Summary

    Malignant external otitis (MEO) in diabetic patients may involve impaired polymorphonuclear neutrophil leukocyte (PMN) function. Vitamin C treatment improved PMN migration and healed ear lesions, suggesting its potential as an immunomodulator.

    Background:

    • Malignant external otitis (MEO) is a severe Pseudomonas aeruginosa infection of the ear canal.
    • MEO disproportionately affects elderly individuals with uncontrolled diabetes mellitus.
    • High mortality rates suggest underlying host defense mechanism deficits.

    Observation:

    • A 58-year-old male patient with MEO presented with impaired polymorphonuclear neutrophil leukocyte (PMN) migration.
    • Standard treatments were ineffective for several months.
    • In vitro studies demonstrated ascorbic acid (vitamin C) improved PMN migration capabilities.

    Findings:

    • The patient received a one-month course of ascorbic acid therapy.
    • Chemotaxis test results normalized concurrently with clinical improvement.
